I am the parent or legal guardian of the children listed on this form. I give permission for them to participate with me in the Fenton Mom Walk at Linden Wood Nature School. Activities may include an optional guided walk on the Discovery Trail, free play at Base Camp, and an indoor activity in the Lodge.

I understand that outdoor walking and play involve risks, including slips, trips, and falls on uneven ground, exposed roots, wet leaves, and slippery surfaces. I understand that the Discovery Trail is not stroller-accessible. I can choose which activities my children and I join, and we may stop or leave an activity at any time.

I understand that this is a parent-accompanied event. I agree to stay with and actively supervise the children listed on this form throughout the event, including on the trail, at Base Camp, and in the Lodge. I agree to follow posted signs and directions from event staff.
